Insurance
I am considered an Out-of-network provider. Out-of-network services are reimbursed at varying rates based on your family’s plan. I do not submit claims to insurance companies. Instead, I provide a detailed superbill that contains CPT codes and other information necessary to submit a claim to your insurance company for reimbursement directly to you.
Payment
An advance deposit of 50% for psychological evaluations is due prior to the assessment date, with the balance due upon completion of the written report. An Intuit Invoice will be sent to your email address to secure payments.
Methods of Payment
Checks, Paypal, Visa / Mastercard / American Express. I also provide “student scholarships” to families in need of financial assistance.
Other Services
I provide consultations and testimony related to my evaluation/written report when appropriate. These services are billed at a set hourly rate and are not reimbursed by insurance.
